Chronic nutritional deficiency present on two evaluations within a consecutive 12-month period and at least 60 days apart documented by 1 or 2: 1. Anemia with hemoglobin less than 10.0 g/dL; or 2. Serum albumin of 3.0 g/dL or less AND

Where claims under 105.08 usually fail

One pitfall is missing the timing rules, such as having the two nutritional-deficiency evaluations less than 60 days apart or not both within a consecutive 12-month period. Another pitfall is using the wrong growth metric for the child's age, for example using weight-for-length cutoffs for a child age 2 or older, or using BMI-for-age cutoffs for a child under 2. A third pitfall is failing the threshold itself, such as having weights or BMI at or above the third percentile rather than less than the third percentile values in the applicable table. A fourth pitfall is mixing up the nutritional-deficiency lab criteria, which must include either hemoglobin less than 10.0 g/dL OR serum albumin of 3.0 g/dL or less, and must be documented on the required two evaluations.
